Parental Waiver/Consent

As the parent or legal guardian of the child/children named during the registration of your child to this camp, and by committing the act of registration, you have given full consent and approval for your child to participate as an attendee in the camp designated above.

You understand that there are certain risks of injury inherent in the practice and play of this sport, as well as in traveling and other related activities incidental to your child’s participation and you are willing to assume these risks on behalf of your child. You hereby certify that your child is fully capable of participating in the designated sport and activities and that your child is healthy and has no physical or mental disabilities or infirmities that would restrict full participation in these activities and that you have disclosed any medical conditions during registration.

In addition to giving your full consent for your child’s participation, you hereby waive, release and hold harmless the organization named above, its officers, coaches, sponsors, supervisors and representatives for any injury that may be suffered by my child in the normal course of participation in the designated sport and the activities incidental thereto, whether the result of negligence or any other cause.
